Patient Privacy Policy

Protected Health Information (PHI) you might provide during an appointment

Name, phone number, address, email address, all dates (except year), medical record number, photos, account numbers.

You have the right to:

Ask to see or get an electronic or paper copy of your vaccination and health medical record.
Ask us to correct health information about you that you think is incorrect or incomplete.
Ask us to contact you in a specific way (for example, home or office phone) or to send mail to a different address.
Ask us not to share certain vaccination or health information, such as care you paid for fully out of pocket, unless sharing is required by state law.
Ask for a list of the times we have shared your vaccination and health information for six years prior to the date you ask, who we shared it with, and why.
Share your vaccination or health history with a medical provider of your choice.
Have someone act as your medical power of attorney. If someone is your legal guardian, that person can exercise your rights and make choices about your health information.
Receive a paper copy of this privacy policy.
File a complaint if you feel your rights are violated. Contact the U.S. Department of Health and Human Services Office for Civil Rights by sending a letter to 200 Independence Avenue, S.W., Washington, D.C. 20201, calling 1-877-696-6775, or visiting www.hhs.gov/ocr/privacy/hipaa/complaints/

Situations your health information may be shared

To submit prescription medication requests to a pharmacy (for example, malaria medication).
If you have given written or verbal permission to share your vaccination or health history with your medical provider, such as sending your vaccination record to your PCP office.
As required or permitted by state law and reporting requirements. For example, vaccines administered at our clinic are reported to the Arizona State Immunization Information System (ASIIS), the statewide immunization registry: azdhs.gov ASIIS page
To run clinic operations such as coordination between medical staff (for example, nurse to physician communication) or to collect outstanding payments.
In response to legal orders and subpoenas.

We will never:

Share your information for marketing purposes.
Sell your information.
Discuss your vaccination or health history with someone you did not approve us to speak with. *Parents or guardians of patients 17 years and younger are assumed to have the right to know their child’s vaccination and health record without the child’s permission.

What this privacy policy covers

This privacy policy applies to the Wildflower Travel & Wellness Clinic website at wildflowerclinicaz.com. It covers how Wildflower Travel & Wellness Clinic (“Wildflower”, “we”, “us”, or “our”) treats personal information collected and received through the website. It also describes the choices available to you regarding our use of your personal information and how you can access and update this information. Personal information is information about you that is personally identifiable, like your name, address, email address, or phone number. Children under 13 are not permitted to use the site, and so this privacy policy makes no provision for children’s use of the site.

Information collection and how it is used

Wildflower collects personal information when you request an appointment, contact us, or complete online health questionnaires. We may ask for your name, email address, phone number, and details about your upcoming travel or visit.

Appointments are scheduled through our practice management and electronic health record vendor, Tebra (formerly Kareo). Any payment or card information you provide when booking or at your visit is handled on Tebra’s secured systems, not stored on this website, and is subject to Tebra’s privacy policy.

Our web server automatically receives and records information from your browser, including your IP address and the page you request. We use this information to provide our services, identify and authenticate users, improve the site, and contact you.

IP addresses

IP addresses are automatically reported by your browser each time you view a web page. We may use them to diagnose technology problems and to estimate the total number of visitors from specific geographic regions.

Cookies and analytics

A cookie is a small amount of data, often including an anonymous unique identifier, that is sent to your browser from a website’s computers and stored on your computer’s hard drive. We use Google Analytics to understand how visitors use the site. Google Analytics sets cookies and collects visit information in the aggregate, such as pages viewed and time on site. We do not use this data to identify you personally. You can opt out with the Google Analytics opt-out browser add-on or by rejecting cookies in your browser settings. If you are a patient, the information you provide when scheduling an appointment online or completing health questionnaires may be considered protected health information (PHI) and will be protected by Wildflower as required by federal and state laws. Our Patient Privacy Policy above describes how we handle PHI.

Information sharing and disclosure

Wildflower may be required to disclose personally identifiable information or protected health information under special circumstances, such as to comply with subpoenas.

We may share aggregate demographic information, such as “most of our patients are in the Tucson, AZ region” or the like. No personally identifiable information or protected health information will ever be used without your permission (for example, we will ask you before using a quote or testimonial).

Wildflower does not rent, sell, or share personal information about you with other people or nonaffiliated companies for promotional purposes, except to provide products or services you have requested or when we have your permission.

Accessing, updating, or correcting your personal information

If your personally identifiable information changes, or if you no longer want to use our services, you may correct it or request deletion by contacting us by phone, email, or postal mail at the contact information listed below. We will respond to your request within a reasonable timeframe.

Security

While we follow generally accepted industry standards to protect the personally identifiable information submitted to us, both during transmission and once we receive it, no method of transmission over the Internet, or method of electronic storage, is 100% secure. While we strive to use commercially acceptable means to protect your personal information, we cannot guarantee its absolute security.

If you click on a link to a third party site, you will leave this site and go to the site you selected. Because we cannot control the activities of third parties, we cannot accept responsibility for any use of your personally identifiable information by such third parties, and we cannot guarantee that they will follow the same privacy practices as us.
